1. Fill out (by hand or electronically) the "HIPAA Authorization" in its entirety and sign/date.
2. Designate on the paperwork whether you wish to receive your records by paper copy or encrypted email.
3. Contact athletic training at 701-671-2309 or sean.owens@ndscs.edu to be sent an encrypted email to electronically return the authorization OR mail the authorization to:
